Consumer and Stakeholder Satisfaction with Council Supported or Conducted Activities

The annual Program Performance Report that the Council prepares and submits to the Administration on Developmental Disabilities (ADD) must include consumer satisfaction and stakeholder satisfaction data. Information about the surveys and the survey forms are included in the information received from the ADD. We would appreciate your taking a few moments to complete either the Consumer Satisfaction Form or the Stakeholder Satisfaction form and submit to us.
