NCCDD Five-Year State Plan Survey Sharing Toolkit

The North Carolina Council on Developmental Disabilities (NCCDD) is planning for its next Five-Year State Plan. The plan will begin October 1, 2027 and end September 30, 2031. Right now, NCCDD is asking for people to complete a survey to help NCCDD find out what should be included in the Plan.

The following is more information about the Plan and the survey. And information on how you can share the survey with other people.

What does the Five-Year State Plan do?

The plan lists the goals and initiatives that NCCDD will support to help improve services and supports for North Carolinians with intellectual or other developmental disabilities (I/DD).


Why should people take the survey?

The survey asks questions about things people need to be able to be a part of their community. The answers will help NCCDD to decide what kinds of projects it will fund to improve the lives of people with I/DD.   


Who can take the survey?

People with I/DD, their families, supporters, and other stakeholders living in North Carolina.


How can I share the survey?

First, share the links for the survey. Surveys are available in English and Spanish. There are three ways to take the survey: online, or by downloading a PDF or Word document. Here are the links to the different surveys.

Second, share with your friends on social media. Are you on social media? If you are, that's one of the best ways to share the survey.

  1. First, download this document for ideas on how to post the information. 
  2. Second, use this image as a photo for your post. To get the image, click on this link: Five-Year State Plan image. When the image appears, right-click on the image, and then save it to your computer.
  3. Third, tag NCCDD in your posts. Remember to use the @ symbol.

Third, send an email to people you know. Email is another great way to share the survey. Download this document for an example of what you can include in your email.
